GP, Pharmacy, Hospital or Mental Health Service.

Your details are held with lots of different health care services.

This has meant that:

  • It can take longer to give you the right treatment
  • You may have to repeat yourself.
  • You may have to remember what care your have been given in the past.

What is the Dorset Care Record?

The Dorset Care Record is a safe way to keep all of your health care details in one place like:

  • Your contact details
  • Your care needs
  • A list of your medical conditions
  • A list of your medications
  • A list of your allergies
  • Test results
  • Any referrals, clinic letters and discharge information
  • History of your treatment

Why do we need it?

We know the health care in Dorset can be better.

The Dorset Care Record will mean that:

  • Health care professionals will be able to use their time spent with you in a better way.
  • Health and Social Care workers can make safer decisions about your care.
