The Dorset Care Record is a safe way to keep all of your health care details in one place like:
- Your contact details
- Your care needs
- A list of your medical conditions
- A list of your medications
- A list of your allergies
- Test results
- Any referrals, clinic letters and discharge information
- History of your treatment
Why do we need it?
We know the health care in Dorset can be better.
The Dorset Care Record will mean that:
- Health care professionals will be able to use their time spent with you in a better way.
- Health and Social Care workers can make safer decisions about your care.

- Health and Social Care professionals will only be able to see your details when they need it.
- You do not have to share your information with the Dorset Care Record.
- You must tell us if you do not want to share your details.
